About the Role

This position has a strong focus on Chronic Disease Management and preventative healthcare initiatives.

Responsibilities include:

• Coordinating GP Management Plans and Team Care Arrangements
• Conducting Care Plan Reviews
• Completing Health Assessments
• Patient education and health coaching
• Recall and reminder management
• Supporting preventative healthcare initiatives
• Working collaboratively with GPs and allied health providers
• Monitoring patient outcomes and follow-up activities
• Maintaining accurate clinical documentation

About You

We are seeking someone who is passionate about patient-centred care and enjoys helping patients achieve better long-term health outcomes.

You will have:

• Current AHPRA registration as a Registered Nurse
• Current CPR certification
• Current COVID-19 Vaccination status in accordance with healthcare requirements
• Experience in General Practice nursing
• Demonstrated experience with:
• Chronic Disease Management Plans
• Care Plan Reviews
• Health Assessments
• Patient follow-up programs
• Excellent organisational and communication skills
• Full Australian working rights
• Experience using Best Practice software will be highly regarded.
